What causes an extreme itch to my cat's ear?

My 10 year old cat started scratching her ear yesterday so hard it started to bleed. Now, today I noticed the inside of her ear is all puffy and swollen looking. She does not seem to be in pain, because she lets me hold her and look at her ear. But she is definitely uncomfortable with the itchiness.
